\u003cp\u003eAt Pathways to Recovery, we believe in a person\u0026rsquo;s right to full inclusion and to a meaningful life of their own choosing, free of stigma and discrimination from the moment you walk in the door. Our program provides psychosocial rehabilitative, recovery-oriented, intensive, community-based services to help members navigate the mental health system and take positive steps toward their recovery. \u003cstrong\u003eIn-Reach\u003c/strong\u003e is a sub-component of PTR and works in conjunction with San Diego County Institutional Case Managers (CICM) to provide case-aide services that are focused on discharge planning and benefit coordination for CICM members residing in long-term care.\u003c/p\u003e

\u003cp\u003ePeer Support Specialists engage, inspire, and facilitate meaningful conversations with members served that assist the person to explore, create, and meet their own recovery goals. Peer Support Specialists provide consultation to the team to promote and reinforce Telecare\u0026rsquo;s Recovery Culture as defined by the Telecare Recovery Centered Clinical System (RCCS), in which each member\u0026rsquo;s point of view and preferences are recognized, understood, respected, and integrated into services and self-help programming. Additionally, Peer Support Specialists respond to critical situations with high level engagement and de-escalation skills which support a least restrictive environment for individuals experiencing an emergency related to a mental health/addiction challenge.\u003c/p\u003e

\u003cli\u003eAssists members with voicing their strengths, interests and goals throughout their services including: goal plans, crisis plans and other methods of expressing individual preferences\u003c/li\u003e
\u003cli\u003eUtilizes a comprehensive resource guide and educates members served about local community supports and resources\u003c/li\u003e
\u003cli\u003eSupports members served in developing strategies to reduce self-stigma\u003c/li\u003e
\u003cli\u003eEngages and communicates with a wide range of members served especially those who are struggling to be active in their own recovery\u003c/li\u003e
\u003cli\u003eAssists members served in all needs including Activities of Daily Living (ADLs)\u003c/li\u003e
\u003cli\u003eActively participates in multidisciplinary team meetings\u003c/li\u003e
\u003cli\u003eParticipates in verbal de-escalation and physical interventions in emergent situations and is willing and able to assist other staff as needed to maintain a safe, healing environment for all\u003c/li\u003e

\u003cli\u003eMust be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment\u003c/li\u003e
