\u003cp\u003e\u0026bull; Two (2) years of inpatient experience in the treatment of individuals with severe mental illness\u003c/p\u003e
\u003cp\u003e\u0026bull; Three (3) months of experience screening psychiatric referrals\u003c/p\u003e
\u003cp\u003e\u0026bull; Must be at least 18 years of age\u003c/p\u003e

\u003cp\u003e\u0026bull; Gathers collateral information following admission to assess social service and treatment needs, including history and current situation.\u0026nbsp; This can include meeting with the member served, family, and team members.\u003c/p\u003e

\u003cp\u003eo Assessing readiness for discharge in coordination with the treatment team\u003c/p\u003e

\u003cp\u003eo Identifying resident discharge planning needs, including arranging for placement, follow-up care and financial aid\u003c/p\u003e
