Available shift: day shift and evening shift to include weekends depending on your availability

• Option A - provide a minimum of four available days each month with no specific holiday or weekend requirement• Option B - provide a minimum of two days available each month and at least one is a weekend shift• Option C - provide a minimum of two available days each month and agree to one holiday annually

Location: Baltimore, MD

Maryland Social Worker license required

This position is in person with no telehealth option1) 1 year of acute hospital care and/or medical case management experience preferred 2) BLS Provider. American Heart Association or American Red Cross3) Case Manager credentialed from the Commission for Case Manager Certification (CCMC)

Responsibilities

Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.

Create plan for care across the continuum, integrating patient/family preference and values.

Monitor patient care through assessment, evaluation and/or patient records.

Advocate for resources and removal of barriers.

Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.

Requirements

Licensure / Certification / Registration:

One or more of the following required:

Clinical Counselor credentialed from the Maryland Board of Professional Counselors and Therapists obtained prior to hire date or job transfer date.

Counselor credentialed from the Maryland Board of Professional Counselors and Therapists obtained prior to hire date or job transfer date.

Social Worker credentialed from the Maryland Board of Social Work Examiners obtained prior to hire date or job transfer date. Any level accepted.

Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.
