The SBCM FSP program provides case management and mental health services with a focus on rehabilitation and recovery. SBCM also has housing subsidies for eligible individuals. The ICM serves individuals who are on Public Conservatorship and reside in a skilled nursing facility or another county-identified long-term care institution.\u003c/span\u003e\u003c/p\u003e
