University Health Services is recruiting a full time Licensed Independent Social Worker to work closely with the clinical team as a Care Manager providing ongoing mental health services to students and supporting staff through planning, implementing, and coordinating resources based on individual student and staff needs. The goal of this position is to promote continuity of care through coordinating, collaborating and advocating based on students' needs. In addition, this position will coordinate care with students receiving services from University Health Services, CAPS, Stetson Psychiatry and area hospitals.
Essential Functions
Assists UHS and CAPS staff with the hospitalization process, discharge planning, follow-ups, and coordination of other services.
Establishes, maintains and coordinates relationships with UC services such as CAPS, Stetson Psychiatry, and other campus organizations that would assist with the students' needs.
Consults with students to determine their needs and best course of treatment.
Provides transitional support to referring therapist and clients in need of referral to community services.
Maintains community referral resources.
Acts as liaison and coordinates between UHS and community referral sources, including hospitals, private practitioners, clinics, psychiatry, and other mental health agencies while maintaining a database of community referral resources.
Work cooperatively with CAPS and Stetson to develop a single access point for mental health services for all UC students.
Required Education
Master's Degree must be in Social Work or related field.
Required Trainings/Certifications
State of Ohio Licensed Social Worker (LISW), or license eligible (must be obtained within six (6) months of employment).
Required Experience
Two (2) years of post-Master's experience.
Physical Requirements/Work Environment
Office environment/no specific unusual physical or environmental demands.
