Job Summary
This position contributes to the mission and vision of CHI Health by assisting in meeting the psycho-social, rehabilitative, financial and discharge planning needs of patients and families requiring continuing care or adjustment following discharge and assisting patients/families with personal or environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care.  Serves as a consultant to Care Managers and the multi-disciplinary team in identifying, organizing and solving problems regarding discharge planning. 
This position reports to the Operations Director, Behavioral and works collaboratively with hospital staff, patients and their families, medical staff and supportive services.  The Social Worker in conjunction with the clinical team participates in the discharge and planning process to ensure patient choice and placement according to patient clinical, psycho-social and financial needs.
 
PRINCIPAL RESPONSIBILITIES:   
                       
Hours: part-time, 10 hours per week


Gathers relevant information systematically; considers a broad range of issues or factors; grasps complexities and perceives relationships among problems or issues; seeks input from others; uses accurate logic and analysis.
 

Relates to people in an open, friendly, accepting manner; uses mission, vision and core  system values in conducting daily contact with patients, families peers, physicians and all stakeholders.
 


Provides group and individual therapy services for program; understands the agendas and perspectives of others; recognizes and effectively balances the interests and needs of one’s own group with those of the broader organization.
 


Speaks clearly and expresses self well in groups and in one to one conversations.
 


Counsels and interviews patients/family members and conducts an appraisal of social, emotional and economic complications in order to provide the physician and the treatment team with recommendations and information which will assist in developing an overall plan for the patient’s care upon consultation or, if functioning, in a selected area.
 


Works with the Care Managers to assure a timely and effective discharge plan hospital utilizing direction from medical staff, and outside agencies which is fiscally responsible.  Documents discharge planning and patient treatment progress in the medical record.
 


Maintains an ongoing therapeutic relationship with patients and families. Provides verbal and written communication; monitors/encourages appropriate and timely progress along the treatment continuum for assigned patients.
 


Collaborates with the health care team; serves as a liaison with community and referring agencies; develops and maintaining good working relationships to assure the availability of quality services. 
 


Responds and intervenes in the resolution of crisis or conflict situations in the inpatient or outpatient setting. Provides therapeutic intervention to patients/families regarding their biopsychosocial needs.
 


  Maintains a system of records and statistics. 
 ','!*!QUALIFICATIONS:
 
Experience
Preferred: Two years acute care experience as a social worker required; experience in a behavioral setting preferred.
 
Education/Licensure:
Required: 
Master Degree in Social Work.
Current Unrestricted Certification to practice as a Social Worker in the State of Nebraska/Iowa.
Certified Master Social Worker (CMSW), Licensed Master Social  Worker – IA (LMSW),
Provisional Master Social Worker (PMSW),
Provisional Licensed Mental Health Practitioner (PLMHP) or Licensed Mental Health                         Practitioner (LMHP).
Crisis Prevention Intervention (CPI) required within 6 months of hire
BLS or BLS instructor certification is required upon hire.  Acceptable credentialing bodies and certifications include the following:  American Red Cross:  CPR/AED for the Professional Rescuer and American Heart Association:  Basic Life Support for Healthcare Providers. 
 
Knowledge/Skills/Abilities:
Attends ongoing continuing education seminars related to social work practice.
Able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.
 Demonstrates knowledge of the principles of growth and development over the life span and possesses the ability to assess data reflective of the patient’s status and interprets the appropriate information needed to identify each patient’s requirements relative to his or her age-specific needs.
Expertise regarding health and supportive services resources. Ability to develop an accurate biopsychosocial assessment and translate assessment information into an active treatment plan in conjunction with collaboration of other team members.','We’re an equal opportunity employer.
