Provides safe, effective and efficient implementation of direct care in accordance with established policies, procedures and standards of care. u003cbr /u003eu003cbr /u003eu003cstrongu003eQUALIFICATIONSu003c/strongu003e u003cbr /u003eRequired: u003cbr /u003e- Graduation from an accredited LVN or LPT program and current licensure are required.u003c/pu003enu003cpu003ePreferred: u003cbr /u003e- Previous experience with population to be served and/or work in a similar program setting preferred. u003cbr /u003e- One (1) year of experience. u003cbr /u003e- Understanding of psychiatric illness, its treatment and knowledge of psychotropic medication. u003cbr /u003eu003cbr /u003eu003cstrongu003eESSENTIAL FUNCTIONSu003c/strongu003e u003cbr /u003e- Demonstrate the Telecare mission, purpose, values and beliefs in everyday language and contact with the internal and external stakeholders. u003cbr /u003e- If employed at Inpatient program, must assist with restraint of clients in the event of assaultive behavior and pass assault crisis/crisis prevention training u003cbr /u003e- If employed at Outpatient program, must evade clients in the event of assaultive behavior and pass assault crisis/crisis prevention training u003cbr /u003e- Establishes and maintains a therapeutic relationship with persons served u003cbr /u003e- Provide nursing care and assist in the welcoming, admission and discharge process u003cbr /u003e- Collects data relevant to person served care. Develops and implements nursing care plans u003cbr /u003e- Demonstrates knowledge of crisis management and intervention. u003cbr /u003e- Transcribes physician orders accurately and in a timely manner u003cbr /u003e- Administers medication and treatments to persons served u003cbr /u003e- Receives and presents change of shift report as assigned u003cbr /u003e- Adheres to medication management policies and procedures u003cbr /u003e- Notifies the prescriber and nursing Supervisor of any changes in behavior or condition. u003cbr /u003e- Documents the persons served progress toward their Recovery Plan goals. ?
