Position Summary
Under the supervision of the Program Manager, provides direct social work case management services for participants enrolled in the Program of All-Inclusive Care for the Elderly (PACE). The Social Worker functions as a participant of the interdisciplinary team and works with participants to increase independence and community integration through a model of self determination. This position has exposure to body fluids and communicable diseases. Majority of time is spent sitting (up to 60% of work time) with frequent standing and/or walking required (up to 40% of work time). The population served consists of adults and geriatrics
Licenses/Certifications Required
Masters degree in Social Work and licensed by Arkansas Social Work Licensing Board
Specific Educational and Skill Competencies Required
Masters degree in Social Work and current Arkansas License/Certification required. A minimum total of one year of personal, clinical/internship, and or employment experience providing care or services for a frail or elderly population required. One to two years case management experience preferred. Knowledge of social work standards of practice and ethics. Experience and knowledge of community resources. Strong interpersonal and communication skills with an ability to work within the team concept.
