University Health Services (UHS) is recruiting a full time Licensed Independent Social Worker to work closely with the clinical team as a Care Manager providing ongoing mental health triage and support to students and assisting staff through planning, implementing, and coordinating resources based on individual student and staff needs. The goal of this position is to promote continuity of care through coordinating, collaborating and advocating based on students' needs. In addition, this position will coordinate care for students receiving services from various on- and off-campus health and wellness partners, including students transitioning into and out of higher levels of clinical care.

ESSENTIAL FUNCTIONS
*Provide clinical triage and mental health support services to students accessing mental health care at University Health Services. *Provide transitional support to referring clinicians and clients in need of referral to community services. *Consult with students to determine their needs and best course of treatment. *Assist UHS clinicians, clinical support staff, and patient service representatives with the psychiatric hospitalization/higher level of care placement process, including coordinating admission & discharge planning, follow-up, and other services as needed. *Act as a liaison and coordinate care between UHS and campus-based clinical and safety partners, including UC Counseling and Psychological Services (CAPS) center, UC Health/UC Physicians Outpatient Psychiatry, on-campus safety resources (UCPD) and on-campus Women Helping Women campus advocates. *Act as liaison and coordinate care between UHS and community referral sources, including hospitals, private practitioners, clinics, psychiatrists, and other mental health agencies while maintaining a database of community referral resources. *Other duties as assigned.
REQUIRED EDUCATION
Master's Degree must be in Social Work or related field.

REQUIRED TRAININGS/CERTIFICATIONS
State of Ohio Licensed Social Worker (LISW), or license eligible (must be obtained within six (6) months of employment).

REQUIRED EXPERIENCE
Two (2) years of post-Master's experience.

PHYSICAL REQUIREMENTS/WORK ENVIRONMENT
